What you'll accomplish
By the end of this guide, you'll have ChatGPT Plus configured to generate high-quality prior authorization appeal letters in under 10 minutes — instead of 45 minutes from scratch. You'll have a reusable system you can use for every PA denial going forward.
What you'll need
Go to chat.openai.com and sign in (or create a free account first). Click your name in the bottom-left corner → "Upgrade Plan" → select "Plus" at $20/month. Enter payment information and confirm.
What you should see: Your ChatGPT interface now shows "GPT-4o" in the model selector at the top. Troubleshooting: If you don't see the upgrade option, try going directly to chat.openai.com/upgrade.
Custom Instructions tell ChatGPT who you are and how you want it to respond — every conversation starts with this context automatically.
Click your name (bottom-left) → "Customize ChatGPT" → "Custom instructions."
In the first box ("What would you like ChatGPT to know about you?"), type:
I am a medical assistant at a primary care physician practice. I frequently need to write prior authorization letters and patient communications. I need professional, formal medical language suitable for insurance companies and for physician signature.
In the second box ("How would you like ChatGPT to respond?"), type:
When I ask for prior authorization letters: Format as a formal letter. Use medical necessity language. Reference clinical appropriateness and patient history provided. Keep to one page. End with a request for expedited review when relevant. When I ask for appeals: cite clinical guidelines or peer-reviewed rationale where applicable. Always note that the physician has reviewed and approved the clinical rationale.
Click Save.
What you should see: These instructions now appear as context in every new conversation. Troubleshooting: If you can't find Custom Instructions, look in the three-dot menu next to your account name.
Open a new chat. In the message box, type and save this prompt somewhere (Notes app, Word doc) to reuse:
Write a prior authorization [request/appeal] letter for the following:
- Procedure/Medication: [what you need authorized]
- Diagnosis: [ICD-10 code and description]
- Patient history relevant to this request: [conservative treatment tried, specialist recommendation, test results, etc.]
- Insurance company: [payer name]

Format as a formal physician letter. One page maximum.
Save this template somewhere you'll find it (a sticky note, your desktop, or a shared Google Doc).
Paste the template into a new ChatGPT conversation and fill in the blanks with a real (or practice) case. Example:
Write a prior authorization appeal letter for the following:
- Procedure: MRI Lumbar Spine without contrast (CPT 72148)
- Diagnosis: M54.5 — Low back pain with radiculopathy
- Patient history: Patient has completed 8 weeks of physical therapy with documented failure to improve. Orthopedic specialist has recommended MRI prior to further treatment planning.

- Denial reason: "Conservative treatment not sufficiently documented"

Format as a formal physician letter. One page maximum.
What you should see: A complete, one-page appeal letter with formal letterhead structure, medical necessity paragraph, clinical justification, and closing request.
What good output looks like: The letter opens with patient and claim information, states the request clearly, explains why the procedure is medically necessary in clinical language, cites the patient's treatment history, and closes with a specific request for reconsideration.
Read the draft carefully. Add any specific clinical details the AI couldn't know (physician name, NPI number, your practice letterhead). Have your physician review and sign before sending.

For peer-to-peer preparation:
I have a peer-to-peer call with [insurer] about a denied [procedure] for a patient with [diagnosis]. Write 5 key talking points I should make to argue medical necessity. Keep them concise and clinically grounded.
For step-therapy documentation:
Write documentation showing step-therapy failure for [medication]. Patient tried [previous drug] for [duration] with [result]. Next step required: [requested medication]. Insurance: [payer].
